Gamifying Exposure Response Prevention (ERP): A Fun Twist on OCD Treatment

Exposure Response Prevention (ERP) is a gold-standard treatment for Obsessive-Compulsive Disorder (OCD), but it can feel overwhelming, repetitive, and emotionally draining. It’s really hard to decide to do something that will make you feel uncomfortable on purpose! What if there were a way to make ERP more engaging—and maybe even fun? That’s where gamification comes in. And it’s for everyone…not just kids!

What Is Gamified ERP?

Gamification means applying game-like elements—points, levels, rewards, and challenges—to non-game settings. In ERP, this could look like:

  • Earning points for each completed exposure
  • Leveling up by facing more difficult triggers
  • Randomizing exposures so that the struggle of “choosing” where to begin is gone
  • Using progress trackers or apps to visualize growth

These strategies can make the recovery process feel more manageable and even empowering.  Instead of approaching exposure work with dread, a sense of excitement about the challenge may present.

Why Gamify ERP?

Gamification isn’t just about fun—it taps into motivation, reward systems, and behavioral reinforcement. For people with OCD, adding structure and incentives to ERP can:

  • Reduce avoidance
  • Boost consistency
  • Make progress more visible
  • Turn setbacks into learning moments

Getting Started with Gamified ERP

There’s no one-size-fits-all approach. Some people use journals or spreadsheets, others build their own systems with friends or therapists (or AI!). You can even find apps designed to track mental health goals in a game-like format (Finch, for example).

Maybe you’re not too into video games and those types of challenges, but you love a good artistic moment–find ways to turn art into exposure, maybe by creating images of a trigger or co-creating so that perfectionism and urge to control are challenged. (Check out our EXPOSED gallery for some art that has been submitted to us). Perhaps you’re more of a writer, and creating an exposure script makes the most sense for you.  Or, maybe a good old fashioned round of Simon Says will help you touch the things OCD needs you to avoid. Perhaps an OCD version of Jenga or a random number generator will help you move past the stuck spots of picking an exposure to get started with.

Once you’ve chosen a format that works for you, dive in and get started! You may be surprised that working to earn the reward you’ve set up for yourself is highly motivating and that, along the way, you recognize a sense of pride in the exposures you’re now able to handle with ease!

Final Thoughts

Gamifying ERP doesn’t replace the hard work—it supports it. Whether you’re just starting treatment or looking for ways to stay on track, adding playful elements might make the journey a little lighter.
